1: Upper lid blepharoplasty, lower lid trans-conjunctival blepharaoplasty. Skin and fat was removed from the upper lids to create a more alert and youthful appearance. Fat that was causing fullness and shadows in the lower lid was removed from behind the eyelid without a skin incision. Note the position of the eyebrows, which are in a neutral position.

2- Upper lid blepharoplasty, lower lid trans-conjunctival blepharoplasty. This is an example of what the same procedure can achieve in a patient with a lower eyebrow position. She preferred to avoid the general anesthesia needed for an endoscopic brow lift. The natural look was created by creating symmetry and a little more depth to the upper lids and by removing the shadow from below.

3: Upper lid blepharoplasty, lower lid trans-conjunctival blepharoplasty. Here is a patient with a naturally high eyebrow position. Her desire was for the ‘biggest’ eyelid platform possible. Again, post-operative symmetry is the most important factor contributing to a pleasing result. The previous three patients demonstrate slightly different results because of different underlying anatomy and patient desires.

In men I am always more conservative and try to simply ‘remove’ a negative, that is, remove the heaviness and fullness that contributes to a tired look. In this man, a small amount of lower eyelid skin was also removed.

5: Upper lid blepharoplasty, lower lid trans-cutaneous blepharoplasty. When the upper lid ‘hooding’ was removed, this man appeared much more awake and alert – but also very natural. At first glance he appears to have ptosis (upper eyelid droopiness where the eyelid tendon also needs to be shortened), but the eyelid levels were fine. Skin and fat only were removed.

6: Asian upper lid blepharoplasty, lower lid trans-cutaneous blepharoplasty. This is an example of removing some upper lid skin and fat and also creating an eyelid crease and fold that did not previously exist. In men, and also in most (but not all) Asian eyelids, the preference is usually for a lower eyelid crease with some retained fullness above the crease. Skin and overactive muscle was removed in the lower lids, reducing the bulge and shadow.

7: Creation of right upper eyelid crease in an Asian woman. This doesn’t look like much, but I created an eyelid crease on the right to match the natural one that existed on the left. It is much more challenging to achieve symmetry when operating on just one eyelid.
