1. Upper lid blepharoplasty, lower lid trans-conjunctival blepharaoplasty. Skin and fat was removed from the upper lids to create a more alert and youthful appearance. Fat that was causing fullness and shadows in the lower lid was removed from behind the eyelid without a skin incision. Note the position of the eyebrows, which are in a neutral position.

2. Upper lid blepharoplasty, lower lid trans-conjunctival blepharoplasty. This is an example of what the same procedure can achieve in a patient with a lower eyebrow position. She preferred to avoid the general anesthesia needed for an endoscopic brow lift. The natural look was created by creating symmetry and a little more depth to the upper lids and by removing the shadow from below.

3. Upper lid blepharoplasty, lower lid trans-conjunctival blepharoplasty. Here is a patient with a naturally high eyebrow position. Her desire was for the ‘biggest’ eyelid platform possible. Again, post-operative symmetry is the most important factor contributing to a pleasing result. The previous three patients demonstrate slightly different results because of different underlying anatomy and patient desires.

4. Upper lid blepharoplasty, lower lid trans-cutaneous blepharoplasty. Men’s eyebrows are naturally lower and flatter than in women. In men I am always more conservative and try to simply ‘remove’ a negative, that is, remove the heaviness and fullness that contributes to a tired look. In this man, a small amount of lower eyelid skin was also removed.

5. Upper lid blepharoplasty, lower lid trans-cutaneous blepharoplasty. When the upper lid ‘hooding’ was removed, this man appeared much more awake and alert – but also very natural. At first glance he appears to have ptosis (upper eyelid droopiness where the eyelid tendon also needs to be shortened), but the eyelid levels were fine. Skin and fat only were removed.

6. Asian upper lid blepharoplasty, lower lid trans-cutaneous blepharoplasty. This is an example of removing some upper lid skin and fat and also creating an eyelid crease and fold that did not previously exist. In men, and also in most (but not all) Asian eyelids, the preference is usually for a lower eyelid crease with some retained fullness above the crease. Skin and overactive muscle was removed in the lower lids, reducing the bulge and shadow.

7. Creation of right upper eyelid crease in an Asian woman. This doesn’t look like much, but I created an eyelid crease on the right to match the natural one that existed on the left. It is much more challenging to achieve symmetry when operating on just one eyelid.

8. Upper lid blepharoplasty, lower lid trans-conjunctival blepharoplasty. This patient had pretty much the same upper lid fullness in her teens. An endoscopic brow lift was suggested, but she desired to avoid general anesthesia. A small amount of fat was removed below. The result has lasted over 18 years. 9. Upper lid blepharoplasty. This patient simply desired to have a little more depth in the upper lids so that she could have a few more options in applying eye makeup. Check out our video testimonial section to hear and see her on the first day and first week after the procedure!

10. Upper lid blepharoplasty. She not only looked less tired and was able to keep her eyelids open more easily, but she also felt about twenty years younger when she looked in the mirror.

13: Upper lid blepharoplasty, lower lid trans-conjunctival blepharoplasty. Again, in men I am more conservative. The fullness in the right upper lid was removed to match the left upper lid and fat only was removed from the lower lids.

14: Upper lid blepharoplasty, lower lid trans-conjunctival blepharoplasty. This is an example of how the brow can slightly descend after upper eyelid surgery only. However, the overall result is still quite an acceptable improvement.

19. Upper lid blepharoplasty, lower lid trans-conjunctival blepharoplasty. The small amount of bulging and shadow of the lower lids remaining after surgery is due to excess tone of the orbicularis muscle. If desired, this can be easily treated with dilute Botox injections to flatten the muscle in this area.

23. Upper lid blepharoplasty, lower lid trans-conjunctival blepharoplasty. This result was achieved without any lower eyelid skin incision. If further improvement of the skin texture is desired, some type of laser skin resurfacing can be considered.

Lower Eyelid Only

Lower eyelid only from behind the eyelid. Since most light comes from above, when the bulge or bag is removed (which is due to eyelid fat) the shadow (or dark circle) goes away.

Lower lid fat removal without a skin incision (transconjuctival blepharoplasty or TCB) When the fat is bulging in the lower eyelid (baggy eyelids) there is often a dark area just beneath it (the so-called dark circles). Since most light comes from above, it hits the bulges and forms a shadow below. No amount of make-up can remove a shadow. Fortunately, as can be seen in these photos, the correct amount of extra fat can be precisely removed through an incision behind the eyelid, avoiding a skin incision entirely. The appearance of the skin will remain the same.

Endoscopic brow lift and upper lid surgery. Note how the natural elevation of the brows creates a less harsh, more open and relaxed look by minimizing the frown lines and the crow's feet.

Endoscopic brow lift, upper lid skin removal and lower lid fat removal. The elevation of the brows in this case uncovers the great underlying eyelid anatomy that was previously hidden by the droopy brow.

Healing - Day by Day

The recovery from eyelid surgery is usually quick and painless. The results are very long-lasting. Here is a unique series of before and after blepharoplasty photos. You can easily see that the rejuvenating effects of the surgery at six weeks is extraordinarily similar to the 19 year post-operative photo.
